Facility Use Request Form

All requests for Church use are subject to consistory approval if necessary. Groups are responsible for their own room set up and take down. They are also responsible for any damage to building or contents.

Persons renting rooms in the church are to leave the room in the same condition as they find it.

Please drop off or send a check to Maurice Reformed Church before the date of rental.

These fees are for usage of the facility, but you are responsible for following the rental procedures posted on the wall (this includes putting away all tables and chairs.)

Fees
-Fellowship Hall - $50             Non-Members - $100
-Multipurpose Room - $35    Non-Members - $80
-Fireside Room - $25              Non-Members - $60
- Youth/activity room - $75    Non-Members - $150
(This includes use of kitchen facilities, and the Family Life Center, if the Center is not being used by a church organization)

- Please return this form along with your payment two weeks in advance
(You can bring it to the office of Emily Borger)
- Checks can be made out to Maurice Reformed Church and paid at the church office.
